I have an acoustic Martin 00cxae Grand concert and the jack that hooks up to an amp has come loose. How do I tighten it back up so that it does not fall out and I can play it with my amp again

You didn't say whether it is just loose or has completely loosened and has fallen into your guitar body....
Guitar jacks are held in place by a nut that screw on over the end of the jack and tighten up to the body. Very simple. Now, if the nut is still there, you simply need a small wrench or even a pair of pliers to tighten the nut up.
Sometimes you need to actually hold the jack tight while you tighten the nut. On electric guitars, that's easy. You can just remove the plate holding the jack and pull it out a little and you can hold it from below.
An acoustic guitar is a bit of an issue. I tried looking at the Martin website for a description or a picture of how your jack is mounted. Couldn't find one. so I will give you the two options.
If the jack is mounted directly on the body, then you can either try to tighten the jack by turning the nut tight, or you need to take off the strings so you can get your hand inside to brace the jack from the inside so you can really tighten it good.
If the jack is mounted on a small plastic plate with 4 small screws, then you can remove the 4 screws and gently pull out the jack assembly just far enough to get a hold of the interior of the jack to hold it while you tighten.
If you have lost the nut completely, then most music stores will have replacement nuts. Most jacks are the same so pretty much any nut made for a guitar jack is fine. Probably cost you a dollar or less.
